Two ways to attend
Concerted Care Group offers the same Intensive Outpatient Program through two delivery options. Both follow the same clinical curriculum, the same schedule structure, and the same standards of care. You choose the format that fits your life, and you can change formats during treatment if your needs change.
In person. Attend group and individual sessions at a CCG center. Best for patients who benefit from routine outside the home, who need on-site medical services or lab work, or who do not have private space or reliable internet.
Virtual, available across Maryland. Attend live video sessions from home. Best for patients managing work schedules, childcare, transportation barriers, or distance from a CCG center. Available to anyone physically located in Maryland during their sessions.
Virtual IOP in Maryland
CCG's virtual Intensive Outpatient Program brings the full IOP experience to patients anywhere in Maryland. Sessions are live and led by the same licensed counselors and clinicians who staff our centers. This is not self-guided content or a chat app. You join a consistent group at a set time, several days each week, and you work with an assigned individual counselor throughout your treatment.
What is included
- Live group therapy using CBT, DBT-informed skills, and relapse prevention
- One-on-one counseling sessions with an assigned counselor
- Psychiatric evaluation and medication management by video
- Medication-Assisted Treatment coordination for opioid and alcohol use disorders
- Recovery planning, crisis planning, and care coordination
- Discharge planning into outpatient care or ongoing support
What you need
- A private space where you can speak freely
- A smartphone, tablet, or computer with a camera
- An internet connection that supports video
- To be physically located in Maryland during each session
Maryland law requires health plans to cover services delivered by telehealth on the same terms as in-person care, and Maryland Medicaid reimburses telehealth at parity with in-person visits. Prior authorization requirements still apply for behavioral health services. Our intake team verifies your benefits before you start so you know your coverage up front.
Who our IOP is for
Our IOP supports adults experiencing:
- Substance use disorders involving opioids, alcohol, stimulants, benzodiazepines, or multiple substances
- Co-occurring mental health conditions including anxiety, depression, PTSD, trauma symptoms, and mood disorders
- Emotional instability, relapse risk, or difficulty maintaining sobriety in early recovery
- Mental health symptoms that need more structure than weekly therapy
- Step-down needs from PHP, residential treatment, detox, or inpatient services
IOP is designed for people who are stable enough to live at home but who need intensive, coordinated support to make progress. If a higher or lower level of care is a better fit, our clinical team will tell you during the assessment and help you get there.
Program structure
IOP meets three to four days per week, typically two to three hours per session. The schedule provides consistent clinical contact, accountability, and structure while you keep working, going to school, or caring for family.
IOP functions as a step-down from higher levels of care or a step-up from standard outpatient therapy. Length of stay depends on clinical progress rather than a fixed number of weeks.

- Psychiatric medication support for anxiety, depression, trauma-related symptoms, and mood instability
- Ongoing monitoring for safety and symptom improvement
Addiction IOP
Our addiction-focused track helps patients build recovery routines, identify and manage cravings and triggers, strengthen relapse prevention strategies, establish sober support networks, rebuild relationships, and reduce high-risk behaviors.

Mental health IOP
Our mental health track supports patients working to reduce anxiety, depression, trauma symptoms, and emotional instability. Treatment builds emotional regulation and distress tolerance, develops healthier thinking patterns and coping strategies, restores daily structure and functioning, and strengthens interpersonal skills.

Where we provide care
Virtual IOP is available to patients located anywhere in Maryland. In-person IOP is available at our centers in Baltimore, Brooklyn, Frederick, Elkton, Cumberland, and Hagerstown in Maryland, and in Dover, Delaware.
Insurance and cost
Medicare and Medicaid are accepted at all CCG locations. We also accept most commercial plans. Coverage for virtual IOP follows the same rules as in-person care under Maryland law, though some plans require prior authorization for behavioral health services. Our intake team verifies your benefits and explains your expected cost before your first session.
