In PRP, individuals are provided skills and support to achieve improved wellness, to enhance the quality of their life, and to pursue the things most important to them. This includes areas such as learning, work, making friends, and keeping relationships. The purpose of the program is to promote recovery, resiliency, and empowerment of the individual. The goal is for individuals to function successfully and live as independently as possible in the community. We offer PRP services to children, adolescents, and adults.

PRP (Minors)
Our Child and Adolescent Psychiatric Rehabilitation Program (PRP) is designed to promote healthy development of a youth’s behavior, social skills, self-management skills, and family relationships. The program is strength-based and empowers the client to attain identified personal goals and objectives.
PRP services are provided in the setting most conducive to enhancing each youth’s own healthy participation in the community. This may include the youth’s home, foster care home, job site, educational setting or other community sites. The intensity of services varies depending upon the youth’s individual needs, environment, and diagnosis.
With the input of the client and family, the PRP team at CCG establishes specific goals. These goals relate to the individual’s areas of personal care, behavioral management, social and other skills. The purpose of the program is to lead the client and his / her family to a positive independent life with less need for health services support.
PRP (Adult)
Our Adult Psychiatric Rehabilitation Program (PRP) is designed as an alternative to and prevention of in-patient hospitalization. Our PRP program addresses the rehabilitative needs of clients with chronic or acute issues impacting their behavioral health needs.
Our clients receive skill development and moderate to intensive support. The program teaches clients to function independently or with a caregiver’s support in residential, educational, and social environments. CCG’s approach allows clients to identify self-directed goals and develop a plan on how to reach their goals.
The rehabilitation team is comprised of a Program Director, Program Coordinator, Licensed Rehabilitation Specialists and Rehabilitation Counselors. Our team ensures a consistent, safe and rehabilitative environment for each client.
We accept Medicaid, Medicare and Self-Pay patients.
